5 Things to Check Before You Choose a Weight Loss Programme in London

You have probably done the sensible things already. You have cut back, counted, walked more, and maybe followed a plan that worked for a few months before the weight crept back. Then you start searching for the top 5 weight loss lifestyle programs in London, and every result promises something different: a new diet, a weekly injection, an app, a coach.

These are questions I hear all the time from my patients. Which one actually works, and who can I trust? My answer is that the name of the programme matters far less than how it is run, and that holds for every one of the top 5 weight loss lifestyle programs in London you will come across. Here are the five things I would check before you commit to anything.

Before you compare the top 5 weight loss lifestyle programs in London

From the outside, most programmes look alike. The websites use the same words and the same before-and-after promises. What separates a good one from an expensive mistake sits underneath:

  1. A doctor leads your care, and you see them face-to-face.
  2. It starts with a full medical assessment, including blood tests.
  3. Medication, if it is used, is part of the plan rather than the whole plan.
  4. Someone stays involved after the first appointment.
  5. You know exactly what you are paying for.

If a programme cannot show you all five, keep looking. (I know this can be hard when you just want to get started!)

A doctor should know your whole medical history

Weight is rarely just about food. Thyroid function, blood sugar, sleep, the medicines you already take and hormonal change in your forties and fifties can all play a part. In perimenopause, for example, weight often moves to the middle even when nothing else in your life has changed, and that phase can last longer than people expect (I have written about how long perimenopause lasts).

That is why my weight loss consultations start with a full medical, metabolic and lifestyle review at 10 Harley Street, with baseline blood tests (charged separately from the consultation fee) and the results explained clearly. I want to understand what is driving the weight before we talk about what to do about it. A form on a website cannot do that, however quick it is.

Those blood tests matter more than people think. I usually look at blood sugar and HbA1c, cholesterol, liver and kidney function, and thyroid, because the results change what I recommend. Someone with early insulin resistance needs a different plan from someone whose weight changed after starting a new medicine, and both need something different from someone whose sleep has fallen apart. If a programme does not ask about any of this, it is guessing.

Before your first appointment, write down three things: the medicines you take, the approaches you have already tried, and the one health outcome that matters most to you. It makes the time far more useful.

Medication should never arrive on its own

GLP-1 medicines such as semaglutide and tirzepatide have changed what is possible for many people, and they can absolutely be used! (I explain the difference between them in Mounjaro vs Wegovy vs Ozempic.) They work best when they are prescribed after a real assessment, started at a low dose and adjusted carefully, and paired with changes to nutrition, movement and strength training.

Here is why that pairing matters. In a large trial of semaglutide, people regained around two-thirds of the weight they had lost within a year of stopping it. That is not a reason to avoid medication. It is a reason to use your time on it to build habits that last, such as protein at every meal and two short resistance sessions a week.

As a general guide, GLP-1 treatment tends to suit people with a BMI of 30 or above, or 27 or above alongside a weight-related health problem, who have already tried lifestyle changes without lasting results. Whether it is right for you comes down to a medical assessment, not a tick-box. And be wary of any programme promising ten pounds in a month: steady loss of around half a kilo to a kilo a week is far more likely to stay off.

Side effects are part of this conversation, too. Nausea, constipation and feeling full very quickly are common in the first weeks, especially as the dose goes up. Most of the time they settle with a slower increase and a few changes to how and when you eat, which is exactly why you want a doctor adjusting the dose rather than a fixed schedule posted to your door.

Muscle is the other thing I watch closely. When weight comes off quickly, some of it can be muscle rather than fat, and muscle is what keeps your metabolism, strength and balance in good shape as you get older. That is why I ask about protein and strength work at every review, not just at the start.

You need someone who stays involved

The first appointment is the easy part. The real work happens in the months after, when the novelty has worn off, the dose needs adjusting, or progress stalls for a few weeks. This is where many programmes, including some that appear on lists of the top 5 weight loss lifestyle programs in London, quietly step back, so ask who you will see at follow-up, how often, and whether it will be the same doctor each time.

A good follow-up is more than a weigh-in. We look at how you are eating, sleeping and moving, how you are getting on with any medication, and whether the plan still fits your life. If work gets busy or something difficult happens at home, the plan should bend rather than break.

In my practice, you see me. Check-ins and progress reviews are part of the plan, so if something is not working we change it rather than waiting for you to give up. Notice the small wins along the way, too: better sleep, steadier energy, a blood pressure reading that has come down. I promise they add up.

Ask for prices in writing as well. My weight loss consultation at 10 Harley Street is £300, which covers your assessment, your plan and writing any prescription you need. Blood tests are not included in the consultation fee and are charged separately, as is the medication itself. You should know that before you book, not after. A programme that is vague about costs is telling you something.

Lasting change usually looks quite ordinary

The changes that hold are rarely dramatic. They tend to look like a breakfast with enough protein to keep you full until lunch, a walk after dinner, going to bed at roughly the same time each night, and lifting something heavy twice a week. None of this is exciting, and that is the point: it fits around a real life, so you keep doing it.

I ask patients to choose one change at a time and stick with it for two to three weeks before adding the next. For example, start with a palm-sized portion of protein at every meal. Once that feels automatic, add a ten-minute walk after your evening meal. Small steps can feel slow at first, but they are far easier to keep than a complete overhaul that lasts a fortnight.

Weight loss is medical care, not a test of willpower

If you have struggled to lose weight, it does not mean you did not try hard enough. Usually it means nobody gave you a plan built around your body, your history and your life. I want us to move away from crash diets, detox teas and prescriptions sold off the back of an online form. Whichever of the top 5 weight loss lifestyle programs in London you are weighing up, the one worth choosing is where a doctor knows you, looks at the whole picture and stays with you until the changes hold. Choose the doctor, not the diet.
